Immunizations Outreach

Concilio, in collaboration with the Philadelphia Department of Public Health, is a key player in the education of families about preventable childhood diseases. By providing immunization education and outreach services to families in the City of Philadelphia, Concilio promotes the healthy growth of children. Outreach activities are geared toward children from birth to 35 months of age who are immunization-delayed according to the KIDS Registry. The goal of the program is to return children to a medical home to receive age-appropriate immunizations.

Concilio was honored for its ongoing 15-year partnership with the Philadelphia Immunization Program. Since 1993, Concilio's Immunization Services component has successfully brought thousands of immunization-delayed children up-to-date with their age-appropriate vaccines.
